Case Management
Case managers coordinate patient care from hospital admission through discharge and post-discharge arrangements. Case managers monitor patients to promote the highest quality of care in the most responsible fiscal manner. They may make referrals for home health, respiratory in-home service, durable medical equipment, personal care and nursing home placement, protective services for children and adults, crisis intervention, children's services, youth health services, VA services, support groups, hospice, Medicaid services, disability services, mental health, nutrition services and Meals-On-Wheels, rehabilitation, indigent drug programs, personal emergency response systems, transportation, financial and other counseling, and other services.
