Your privacy is important to us – that’s why we take extra measures to ensure that only you can access your medical information through Access My Chart.
To further ensure that we adhere to federal patient privacy regulations, our hospital sets an age limit that prevents primary account holders from viewing or accessing a dependent patient's health information. The age at which this occurs for our hospital is 13. This pertains to children, spouse and elderly dependents. Therefore, patients over the age of 13 are encouraged to register as a primary account holder. We understand that there are instances where a dependent relationship with a person over the age of 13 is appropriate; with written consent and identity verification, these types of relationships can be approved. If you have the legal authority to access an adult dependent’s health information, contact us at support@accessmychart.com or call 304-637-3882 to receive a consent form that will require the signatures of both the primary account holder and the adult dependent.
