Health Coaching Service

Our Population Health Coach provides one-on-one education to patients that have two or more chronic diseases. This care navigation helps patients gain the knowledge, skills, and gives them access to all of the services available within Davis Health System to become active participants in their care so that they can reach their self-identified health goals.

Health Coaching services include:

• Transitional Care Management
• Chronic Care Management (Medicare)
• Medicare Annual Wellness Visits
• Providing goal-based wellness coaching
• Develop customized plan of action for care
• Coordinate care with all providers
• Accountability & Motivation
• Informational and educational resource
• A compassionate care partner
